47 year old man presents with nausea and is found to have a cerebellar lesion. No frank cerebellar signs. Included in no particular order is: FLAIR, DWI, ADC, and post contrast T1W image. Do you biopsy?
The answer is no!!! This is a PICA stroke. Clearly DWI positive with a adc dark. The lesion appears to spread past midline due to the fact that there is swelling. correct management is to reimage
